Victor Community Support Services is seeking a Family Support Counselor to provide ongoing support to youth and families in residential and community-based programs. The role includes referrals, advocacy, case management, and coordinating with treatment teams to implement individualized plans.

The position requires a high school diploma or GED, with preference for prior direct care experience. Travel to client homes and community locations is expected; a valid driver’s license is desirable.

Qualifications

• Must possess a high school diploma/GED. No previous experience required.
• 60 units of college, or four years’ experience, or Bachelors’ degree.
• Crisis and case management experience is helpful and desired.
• Strong analytical and problem solving skills.
• Previous experience working with the public and government agencies.

Responsabilités

• Provides individual and family support services per individualized plans and may include referrals, advocacy, linkage and case management, support groups, education, transportation, home management, recreational activities, and behavioral interventions.
• Engages with clients with appropriate boundaries and maintains relationships.
• Manages a flexible community-based schedule.
• Completes documentation (forms, notes, data tracking) within required timeframes.
• Participates in treatment teams and liaises with community resources.
• Meets service percentage standards and supports additional program tasks as directed.
