Job summary

Home Base, Inc. seeks an Outreach Counselor to support families and at-risk children, delivering hands-on social work experience in in-home services and community-based interventions.

Qualifications include four-year degrees in Social Work or Human Services and the ability to pass background checks; a valid driver’s license is required, and evenings/weekends may be necessary. Qualifications

• Requires four-year degree in Social Work or Human Services.
• Master’s in Social Work preferred but not required.
• Must pass Fingerprint Background Check (CIB) and APS/CPS checks.
• Valid Driver’s License and Insurance; own reliable transportation.

Responsibilities

• Establish personalized service plans and provide interventions.
• Schedule weekly client visits and coordinate with DHHR case managers.
• Document all contacts and keep chronological intervention notes.
• Present updates and recommendations during Multi-Disciplinary Team meetings.
• Testify in court regarding client behavior and progress.
• Promote respectful communication and professional conduct.
