Job summary

River Valley Family Health Centers in Delta, Colorado is seeking an experienced Advanced Practice Provider (NP/PA) to join our clinics. The APP delivers comprehensive primary care, managing acute and chronic conditions, preventive care, tests, medications, and in-office procedures as credentialed.

This full-time exempt role emphasizes team-based care, care coordination, patient education, and commitment to access for underserved populations. Travel between clinics is required.

Qualifications

• Active Colorado PA-C Certification with NCCPA or FNP license and DEA registration.
• Ability and willingness to treat MOUD.
• 5+ years of related experience in FQHC or community health setting preferred.
• Current BLS/CPR and ACLS certification.
• Ability to travel between clinics.

Responsibilities

• Provide direct patient care and develop management plans.
• Perform injections, suturing, wound care and minor procedures.
• Educate patients on chronic care, prevention and health maintenance.
• Collaborate with Behavioral Health, Primary Care and Dental teams.
• Support continuity of care across clinics.
