Job summary

River Valley Family Health Centers is seeking an experienced Advance Practice Provider to join clinics in Delta, Colorado. The APP will deliver patient-centered primary care, manage acute and chronic conditions, and perform in-office procedures while coordinating care with the interdisciplinary team.

The role is full-time, salaried exempt, based in Montrose, CO, with compensation ranging from $118,779 to $136,011.20 per year, depending on experience and certifications.

Qualifications

• Active PA-C certification with NCCPA or current FNP license in Colorado, plus DEA registration.
• Ability and willingness to treat and prescribe MOUD.
• Comfort with treating patients of all ages, including infants, with well-child exams preferred.
• Current BLS/CPR and ACLS certifications.

Responsibilities

• Provide direct patient care including health history, exams, and care plans.
• Perform therapeutic procedures such as injections, suturing, wound management.
• Educate patients on chronic care, preventive care, and healthy lifestyles.
• Collaborate with Behavioral Health, Primary Care and Dental teams for integrated care.
• Travel between clinics as needed and maintain timely follow-up.
