Job Summary

Within an outpatient ambulatory practice setting, within thecontext of a clinical care team and a Patient Centered Medical HomeResidency practice, provide full spectrum ambulatory careincluding, but not limited to, chronic disease management, acuteand “urgent” care, and wellness and preventative care for adult andpediatric patients. The Advanced Practice Provider (APP) evaluates,assesses, diagnosis, prescribes and educates patients regardingtheir particular illness or chronic disease. The position provideshigh quality primary care to all patients, evaluate and treat acuteand chronic problems for the patient population, completehistories, thorough assessments and physical examinations on allpatients with accurate and complete documentation reflected inmedical records and collaborates and communicates with otherdepartments and colleagues, such as the pharmacy, to ensure optimalpatient virtual care, including via telehealth appointments.

TOTAL FTE COMMITMENT

1.00 cFTE ( 32 hours of appointable clinic hours per week and 8 hours per week of clinicaladministrative time (paperwork, documentation, required meetings,required trainings, etc.).)
