Job summary

Easterseals Hawaii seeks a Care Coordinator (EI) to support families of infants and toddlers, coordinating services and monitoring progress toward developmental goals.

The role partners with families, clinicians, and state EI staff to implement the Individualized Family Service Plan, deliver home visits or telepractice, and ensure timely documentation and funder compliance. In-person office presence 4–5 days weekly.

Qualifications

• Bachelor's degree in Social Work or related field.
• Active LSW or LCSW license required.
• One year of professional social work experience preferred.
• Experience with children with special needs in early intervention preferred.

Responsibilities

• Coordinate development monitoring and intervention services for 0-3 yrs with delay or at risk.
• Educate and coach families and care providers to support child development.
• Collaborate with team to develop and monitor child outcomes on IFSP.
• Coordinate with Hawaii DOH EI and meet funder guidelines and reporting.
• Meet caseload productivity and provide intervention strategies and resources.
• Schedule weekly Family Support Team meetings under supervision.
• Arrange home or telepractice visits based on family preference.
• Coordinate family visits with other providers as needed.
• Assess and consult on child's progress using evidence-based tools.
• Complete billing and service plan documentation within timelines.
• Communicate recommendations and materials to families and care providers.
• Explain Primary Service Provider Coaching Model and support families.
• Provide referrals to additional services as needed.
• Stay current with EI research, trends, and related fields.
• Perform additional duties as assigned; report in person 4–5 days/week.
