Job summary

Johnson County Developmental Supports (JCDS) is seeking a Service Coordinator to assess individuals, develop plans, and coordinate resources in a community setting. JCDS serves people with intellectual and developmental disabilities and emphasizes choice-driven supports and advocacy to live and work in the community.

The role involves coordinating assessments, plans, transportation, and services for a caseload, with documentation and progress evaluations to meet measurable goals.

Qualifications

• Bachelor's degree in Human Services or related field and one year of related work experience are required.
• KS/MO driver's license may be required, depending on department or agency.
• Reference/background screening is required for all positions.

Responsibilities

• Conducts assessments of individuals' background and applicable circumstances to identify needs and determine service eligibility.
• Develops plans to address the needs of individuals and coordinates resources with community agencies.
• Provides services for a designated caseload, transports individuals as needed, and facilitates skill development and support groups.
• Completes and submits documentation; evaluates progress and reviews service quality.
