• Represent the MRCSB case management program with various federal, state and local entities.
• Maintain an accurate record keeping system and documentation per regulatory standards.
• Assess eligibility for DD/ID services and develop ISP-based plans.
• Coordinate services, monitor ISP implementation, and adjust plans as needed.
• Identify risks and lead planning to mitigate them over time.
