Job summary

CVS Health, through Omnicare, seeks a Consultant Pharmacist to support long-term care facilities across Indiana. The role emphasizes clinical reviews, regulatory compliance, and program adoption while collaborating with facility leadership and CVS teams.

You will perform Medication Regimen Reviews, use OSC2OR and MDS 3.0 tools, and travel to client sites. A BS/PharmD and Indiana license are required, with strong LTC experience and immunization credentials preferred.

Qualifications

• Residency or licensure to practice pharmacy in Indiana
• 3+ years of clinical pharmacy experience preferred
• Immunization certification or willingness to obtain it
• Certification in geriatrics and/or gerontology preferred

Responsibilities

• Ensure facilities remain compliant with federal regulations and state requirements
• Conduct clinical reviews and document findings per regulatory requirements
• Lead medication regimen reviews and provide written reports to facility staff
• Collaborate with account management, sales, and operations to meet site goals
• Educate facility staff on new programs and processes implemented by Omnicare
