Job summary

Bon Secours Mercy Health in Lorain is seeking a Community Health Program Coordinator to support the planning, execution, and promotion of community health initiatives. You will coordinate with internal departments, partners, and local organizations to deliver programs that reach diverse populations in the Lorain service area.

The role requires event participation, local travel, and occasional evenings/weekends for community events and department initiatives.

Qualifications

• High school diploma or equivalent required.
• Associates or Bachelor’s degree in relevant field preferred.

Responsibilities

• Assists with planning and implementation of community health programs in support of Community Health goals.
• Participates in outreach to local organizations, community members, and care providers to promote participation and awareness of health services.
• Coordinates operations and support related to planning, communication/promotions, logistics, and documentation of community health initiatives to reach diverse populations.
• May collect, track, and report program data to leadership to support continuous improvement and compliance.
• Provides input into policies and programs that improve community health delivery and interventions.
