Job summary

Mercy Health's Lorain Hospital seeks a Community Health Program Coordinator to support implementation of community health programs and collaborate with partners across Mercy Health, government agencies, faith-based and non‑profit groups to ensure success.

The role involves planning, outreach, logistics, and data reporting to leadership to improve health services and promote community wellness. Full-time, in-person position with evening/weekend events.

Qualifications

• High School Diploma or equivalent is required.
• Associate’s or Bachelor’s degree in a relevant field is preferred.
• 1 year of related experience is required; 3 years preferred.
• Bilingual based on market needs is preferred.

Responsibilities

• Assist with planning and implementation of community health programs.
• Engage with local organizations, community members, and care providers to promote participation.
• Coordinate operations, logistics, communications, and documentation of initiatives.
• Collect, track, and report program data to leadership to support improvements.
• Contribute input to policies and programs to improve community health.
