Job summary

Clarvida is seeking an Enhanced Community Health Worker (ECHW) in California to provide culturally responsive, community-based support for individuals with behavioral health needs enrolled in SMHS/DMC programs. You will connect clients with resources, promote engagement, and advocate within care teams.

The role blends field and office work, requires lived experience and a CHW certificate (or equivalent), strong communication, and ability to navigate complex systems.

Qualifications

• Demonstrated lived experience reflective of the populations served in behavioral health.
• Completion of a Community Health Worker certificate program or equivalent.
• Strong communication and relationship-building skills.
• Knowledge of community resources and service systems.
• Ability to work with diverse, high-acuity populations.
• Understanding of trauma-informed, culturally responsive, person-centered care.
• Basic computer skills and ability to use EHR systems.
• Valid driver’s license and reliable transportation if field work is required.

Responsibilities

• Conduct outreach and engagement with individuals facing behavioral health challenges.
• Build trust using culturally responsive, trauma-informed approaches.
• Support re-engagement and navigation across services and systems.
• Provide education on mental health, wellness, and healthy lifestyles.
• Assist with scheduling, referrals, transportation, and follow-up tasks.
• Document services and outcomes in the electronic health record per requirements.
• Collaborate with clinicians and care teams to coordinate care.
• Participate in team meetings and care coordination activities.
• Perform additional duties as assigned.
