Job summary

InterCommunity, Inc. is a Federally Qualified Health Center Look-Alike delivering accessible primary care and behavioral health services across multiple community health centers.

The Registered Nurse will collaborate with the Clinical Practice Manager and multidisciplinary teams to ensure high-quality patient care. The role requires CT RN licensure, BLS/CPR, and 1–2 years in public health or related fields.

Qualifications

• Graduation from an accredited nursing program.
• Current State of Connecticut RN licensure.
• One year medical experience in a public health setting.
• Two years of community health, addiction medicine and/or psychiatric nursing.
• BLS/CPR certification.
• EPIC experience preferred.
• Bilingual (Spanish Speaking) preferred.

Responsibilities

• Provides direct patient care in primary care and psychiatry.
• Carry out orders from medical providers including history, vital signs, phlebotomy, EKGs, wound care, and medication administration.
• Conduct nursing visits or educational groups as ordered.
• Maintain accurate and detailed records.
• Monitor and report patient conditions and medication responses.
