Job summary

Lutheran Social Services of WI & Upper Michigan is seeking a full-time Care Coordinator to support children and families in Ozaukee County through the CLTS Waiver Program. You will serve as the primary contact, complete assessments, identify service needs, and develop and coordinate individualized, family-centered service plans to help children thrive at home and in their communities.

This hybrid role offers up to two remote days per week, Monday–Friday first-shift hours, and a comprehensive

Qualifications

• Bachelor's degree in Social Work or related Human Services field.
• At least 1 year of experience working with children and/or youth with disabilities or equivalent education/experience.
• Valid driver's license and reliable transportation.
• Strong communication, organization, and documentation skills.

Responsibilities

• Serve as the primary point of contact for CLTS participants.
• Complete assessments and identify service needs.
• Develop and coordinate individualized, family-centered service plans.
• Connect families with community resources and providers.
• Facilitate team meetings and collaborate with service partners.
• Maintain accurate documentation and client records.
• Provide trauma-informed, person-centered support.
