Jewish Family & Children’s Service of Arizona is seeking a compassionate Case Manager for Children's Behavioral Health in the Valley. The role provides targeted case-management for children and guidance to families/caregivers to support stability at home and in the community.

The position requires a high school diploma or GED with related behavioral health experience, or a degree in social work/psychology, plus a valid AZ driver’s license and clearance.

Qualifikationen

• Requires 4 years of behavioral health experience (HS diploma or GED).
• A.A. degree with 2+ years behavioral health experience acceptable.
• Bachelor’s in Social Work, Psychology, or related field preferred.
• Must have Level 1 Fingerprint Clearance Card without Restriction(s).
• Meet agency requirements for MVR (Motor Vehicle Record).
• Must have reliable transportation and AZ driver’s license.

Aufgaben

• Provides weekly contact with assigned clients, including home/community visits and medication checks.
• Assists youth in navigating discharge from treatment settings and finding clinically indicated services.
• Coordinates services with agencies (DCS, Probation/Parole, DDD, schools) and other providers.
• Coordinates and facilitates Child & Family Team (CFT) Meetings.
• Collaborates with clinical team to ensure Integrated Service Plans, CFT notes, and paperwork are completed accurately and timely.
