Qualificações

• Bachelor’s degree in Social Work required.
• MSW or CCM preferred.
• Active, unrestricted SW license.
• Minimum 3 years of Social Work experience with older adult population in healthcare.
• 3-5 years of clinical experience with 1-3 years of case management review.
