You will develop beneficiary-centered care plans, collaborate with providers, and pursue ongoing education while maintaining strict confidentiality and regulatory compliance.

Qualifications

• Bachelor's degree in Social Work required.
• MSW or Case Management Certificate (CCM) preferred.
• Active, unrestricted SW license.
• Minimum 3 years of Social Work experience with older adults in a healthcare environment.
• 3-5 years of clinical experience with 1-3 years of case management review.

Responsibilities

• Assess, plan, implement, monitor, and evaluate care options across the continuum.
• Use clinical knowledge and communication skills to achieve optimal beneficiary outcomes.
• Develop beneficiary-centered care plans.
• Coordinate care with beneficiaries, providers, and team members.
• Address gaps in care or compliance and maintain confidentiality.
