Qualifications

• Bachelor's degree in Social Work or related field.
• Zero years of experience with a Bachelor's degree.
• Knowledge of SCS programs and services, policy and procedure.
• Good interpersonal skills and written and oral communication skills.
• Familiarity with applicable regulations and licensure standards.

Responsibilities

• Assess client needs and participate in development of service plans.
• Arrange delivery of needed services identified in assessment.
• Assist clients in accessing services via referrals or linkages.
• Monitor progress through referrals, appointment tracking and follow-ups.
• Advocate for clients, e.g. letters to expedite entitlement payments.
• Prepare and maintain case records per standards.
• Provide case consultation with service providers.
• Communicate with supervisor about case management issues and resources.
• Provide psychosocial education to clients/families.
• Perform crisis assistance and emergency referrals.
• May serve as Primary Case Coordinator depending on skill and autonomy.
