Service Core inc. is seeking a Case Manager to provide direct mental health and SUD support, including needs assessment, ISP formulation, and progress documentation.

The role requires travel to clients’ appointments in a company vehicle, with about 75% travel, and working within agency standards and risk management protocols. The ideal candidate has at least 3 years of mental health or community support/case management experience, knowledge of community resources, and proficiency with Electronic

Competenze

• 3 years mental health/ SUD or community support/case management experience required.
• Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis.
• Knowledge of community resources is critical.
• CDCA is required if you do not have a Bachelor’s degree; must obtain within 45 days of hire.
• Experience using Credible EHR and standard office tools.

Mansioni

• Conduct needs assessment and formulate ISP; provide crisis stabilization and supportive interactions.
• Decide on additional services, resources, or settings and coordinate advocacy and collateral contacts.
• Complete intake/needs assessment, ISP reviews, progress notes, Payeeship and discharge paperwork.
• Ensure compliance with agency, ODMH, CARF and payer standards and obligations.
• Engage with peers, supervisors, other providers, and families to support care progress.
• Participate in agency meetings and supervision; implement process improvements.
• Share knowledge and skills with colleagues; pursue ongoing education; expand competencies.
