Job summary

Sertoma Star Services is seeking a Case Manager to support individuals with intellectual/developmental disabilities and mental illness in the community. The role focuses on community integration, skill generalization, and linking consumers to essential resources during recovery.

You will handle intake screenings, scheduling, transport coordination, and documentation within Rule 132 guidelines, while advocating for consumer needs and rights in a supportive, team-based environment.

Qualifications

• Bachelor's degree in psychology, counseling, social work, or related field within six months of hire.
• Minimum of one year experience working with individuals with mental illness.
• One year case management, linkage, outreach, and/or community support experience preferred.
• Experience with Medicaid Rule 132 documentation and billing requirements preferred.
• License/Certification – Current license in counseling or social work preferred.

Responsibilities

• Directly deliver services per Rule 132 program requirements.
• Schedule and document all billing activities to meet deadlines.
• Transport consumers in accordance with agency policy.
• Maintain consumer records in the Electronic Clinical Record.
• Coordinate transportation and link consumers to resources.
