Job summary

Gosnold is seeking a BH-JI/CSP Case Manager with a foundation in mental health and addiction recovery. The role involves conducting patient interviews, establishing care plans, and coordinating with community resources to support recovery. Experience with the criminal justice system is beneficial, and a MA driver’s license is required.

Applicants should be prepared to work in a multidisciplinary team, manage a caseload of ~17–20 patients, and track tasks using a CRM system.

Qualifications

• Education: Bachelor's degree in counseling, psychology, social work, nursing, or related discipline required.
• Experience: Two years of either professional or lived experience in mental health and addiction.
• Skills: Strong interpersonal and organizational skills; proficient with web-based EHRs.

Responsibilities

• Conduct patient interviews, complete basic assessments, and engage patients into treatment and continuing care planning.
• Develop case management service plans and care plans with patients and supports.
• Coordinate with community resources and agencies to support recovery goals.
• Maintain records in compliance with Gosnold standards and regulatory requirements.
• Collaborate with multidisciplinary team and participate in supervision and training.
