Job summary

Gosnold, Inc. is seeking a BH-JI/CSP Case Manager in Massachusetts to support mental health and addiction recovery services. You will interview patients, develop care plans, and coordinate community resources while working within correctional settings.

The role requires a bachelor's degree in counseling, psychology, social work, or related field, plus at least two years of relevant experience, and a valid MA driver’s license. This is a full-time, on-site position with comprehensive benefits.

Qualifications

• Bachelor’s degree in counseling, psychology, social work, nursing, or related discipline required.
• Two years of either professional or lived experience in mental health and addiction.
• Strong interpersonal and organizational skills; comfortable with the justice system and correctional settings.
• Valid MA driver’s license.

Responsibilities

• Conduct patient interviews, complete basic assessments, and engage patients into treatment and continuing care planning.
• Conduct case management service plans.
• Some psychoeducation groups may be conducted within correctional facilities.
• Prepare care plans in collaboration with patients and supports.
• Establish connections and utilize community resources for recovery support.
• Develop and manage systems to comply with regulatory standards.
• Accompany patients to resources as needed per service plans.
• Assist patients with transportation and other resource needs.
• Participate in staff supervision and in-service training.
• Attend team meetings to review patient progress.
• Communicate with referral sources and community resources.
• Maintain patient records per Gosnold standards.
• Work well on a multidisciplinary team.
• Fulfill outreach functions with own transportation.
• Perform other duties as assigned.
