Job summary

Depaul USA in Pine Bluff, AR seeks a part-time Case Manager for the Opportunity House program. You will conduct client assessments, develop service plans, coordinate care, and assist residents toward permanent housing.

Qualifications

• Minimum of an associate’s degree in related field (e.g. social work, psychology, etc.); 1 year work experience in related field
• Demonstrated knowledge of core social work principles, ethics, and case management practices, including assessment, service planning, documentation, resource coordination, advocacy, and maintaining appropriate professional boundaries
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint), and experience with data management systems

Responsibilities

• Outreach/screen potential clients for eligibility using ESG guidelines
• Complete intake assessments and service plans with clients
• Track client enrollment and prepare reports for grants
• Refer clients to housing, social services, and employment resources
• Coordinate care with other service providers and supports
