Job summary

Options for Community Living, Inc. is seeking a Case Manager to join our team in Ronkonkoma, NY. The role provides supportive case management to individuals with mental illness in permanent housing, with field visits and site monitoring.

An ideal candidate will have a high school diploma or higher, experience in case management, a valid driver’s license, and the ability to work with vulnerable populations on Long Island. Bilingual English/Spanish preferred.

Qualifications

• Qualifying experience: verifiable full or part-time case management or case work with persons with a history of mental illness, homelessness or chemical dependence.
• Excellent time management skills.
• Bilingual preferred (English/Spanish).

Responsibilities

• The case manager provides supportive case management services to individuals with mental illness in permanent housing.
• Monitors maintenance of sites and provides on site visits to scattered housing locations.
• Plan and evaluate service plans and monitor objectives; write progress notes bi-weekly.
• Perform site visits 1-3 times weekly depending on client need.
• Provide information regarding community resources and orient residents to the area.
• Assist in securing vocational assessment services, job training and education programs.
• Provide advocacy services to gain entitlements and services.
• Intervene in situations requiring immediate attention to ensure safety of residents.
• Oversee medication maintenance and assist residents with self-medication skills as needed.
