Job summary

Options for Community Living, Inc. is looking for a dedicated Case Manager to provide supportive case management services to individuals with mental illness. The ideal candidate will have a High School Diploma or a Bachelor’s degree along with relevant experience.

Qualifications

• High School Diploma and 2 years of qualifying experience, or a Bachelor's degree and 1 year of qualifying experience.
• Valid driver’s license, safe driving record, valid auto insurance, and access to a vehicle.
• Verifiable experience in case management with persons who have mental illness or homelessness.

Responsibilities

• Provide supportive case management services to individuals with mental illness.
• Conduct on-site visits to scattered housing locations.
• Write progress notes weekly and monitor objectives.
