Job summary

Options for Community Living, Inc. is seeking a Residential Support Case Manager to join our Mobile Residential Support Team on Long Island. The ideal candidate will provide case management services to tenants with mental illness in supportive housing and collaborate with the treatment team.

This in-person role is based out of our Ronkonkoma office with field visits required. Schedule options total 37.5 hours per week, typically Monday through Friday with various start times.

Qualifications

• High School Diploma or equivalent and two years of experience in case management with disabled or economically disadvantaged individuals.
• Bachelor's degree in human services related discipline or two years of college in a human service related discipline is preferred.
• Valid driver’s license, safe driving record, valid auto insurance and access to a vehicle is required.

Responsibilities

• Provide site visits and write progress notes with tenants (when possible) and review service plans and goals.
• Provide linkages to community resources.
• Assist in securing vocational assessment services, job training and education programs.
• Provide advocacy to gain entitlements and services.
• Intervene in situations requiring immediate attention to ensure safety of residents.
• Provide mediation, counseling, behavioral intervention and crisis management services.
• Consult and coordinate with clients’ family and providers as needed for coordinated service delivery.
• Notify Program Supervisor of any medical or psychiatric emergency.

Salary Range: $42,900/year ($22.00/hour) - $54,600/year ($28.00/hour)

The above salary range represents Options for Community Living’s good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. When determining a salary offer, several factors may be considered as applicable (e.g., years of relevant experience, education level, language skillset, credentials, professional licensure, budget, and internal equity).

Schedule Options: (37.5 hours/week)

• Monday - Friday: 8:00 AM - 4:00 PM (30 minute break)
• Monday - Friday: 8:00 AM - 4:30 PM (1 hour break)
• Monday - Friday: 8:30 AM - 5:00 PM (1 hour break)
• Monday - Friday: 9:00 AM - 5:00 PM (30 minute break)

Location: In person based out of our Ronkonkoma office, with field visits required.

Pay Type: Non-exempt

Responsibilities:

The Residential Support Case Manager provides supportive case management services to tenants with mental illness in supportive housing and provides on-site visits to scattered site locations. The Case Manager will work with the tenant, treatment team, and the Mobile Residential Support Team (MRST) to identify issues and barriers that may prevent the individual from maintaining stability in the community. Through a mutually agreed goal plan, the MRST Case Manager will assist the tenant in seeking solutions to achieve mental health and health care goals, sobriety, housing stability and to prevent where possible, emergency room usage and inpatient hospitalization.

• Provide site visits, write progress notes collaboratively with the tenant (when possible), review service plan and goals.
• Provide linkages to community resources.
• Assist in securing vocational assessment services, job training and education programs.
• Provide advocacy services to gain appropriate entitlements and services.
• Intervene in situations requiring immediate attention to ensure safety of residents.

• Notify Program Supervisor of any medical or psychiatric emergency.
