Job summary

Acenda Integrated Health in New Jersey is hiring a Care Manager hourly role to provide support services and case management for families facing homelessness or housing instability. You will coach parents on life skills, facilitate access to community resources, and work with care teams to advance individual and family stability through home visits and collaborative planning.

Starting base rate $19.23 with a $500 sign-on bonus; bilingual rate adds $1.50 when Spanish proficiency is required.

Qualifications

• Bachelor's level practitioner with at least 2 years of experience in social service-related field preferred.
• Valid driver's license with an acceptable driving record.
• Ability to exercise independent thinking, good judgment, and effective time management.

Responsibilities

• Serve as the contact point and advocate for clients and care team.
• Monitor adherence to service plans and evaluate progress.
• Facilitate linkages to treatment and community supports.
• Maintain a caseload of 10–12 families with in-home sessions.
• Educate families about community resources.
• Build and maintain relationships with external providers.
• Submit accurate reports and documentation on time.
• Ensure clear, high-quality written and verbal communications.
• Support the Quality/Compliance/Risk functions as needed.
• Perform other duties as assigned.
