Qualifications

• Bachelor's or Master's degree in psychology, social work or related field; plus 2 years of relevant experience.
• Must have and maintain a valid driver’s license with an acceptable driving record
• Must be able to exercise independent thinking and good judgment under all circumstances.
• Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Responsibilities

• Care Coordination: Oversee and manage client care plans, ensuring timely access to services and resources.
• Collaboration: Work with providers, clients, and families to develop comprehensive care plans.
• Monitoring Progress: Regularly assess and monitor client progress and adjust care plans as needed.
• Client Advocacy: Advocate for clients’ needs to ensure high-quality care.
• Documentation: Maintain accurate case documentation and client records in a regulatory-compliant manner.
• Adheres to a flexible schedule based on client and program needs.
