Job summary

Brunswick County is seeking a Case Manager to join the Emergency Medical Services team, supporting Community Paramedic Services. The role focuses on connecting clients with resources to remain at home and address health needs, including substance use, while collaborating with team members and providers.

The Case Manager will manage documentation, work independently, and report to the Community Paramedic Services Program Supervisor.

Qualifications

• Bachelor's degree in social work from an accredited institution, OR a related human services degree with 1–2 years related experience.
• Valid NC or SC Driver License required.
• Proficient in Microsoft Word and Excel.

Responsibilities

• Facilitate needs and care options for community paramedic patients.
• Liaise between community paramedics and providers.
• Monitor patient progress.
• Identify and connect patients to services.
• Advocate for and educate individuals and family members.
• Assist with the Access and Functional Needs Program planning, development, implementing, and managing countywide activities during disasters.
