Job summary

Sweetser in Brunswick, Maine offers a Case Manager–CCBHC role to support children, adults, and families. You will conduct assessments, coordinate care with an interdisciplinary team, and help clients access health services, housing, and basic needs within a structured care continuum.

The role emphasizes documenting services in the EHR and coordinating crisis intervention when needed, with MHRTc certification preferred and a valid driver’s license required.

Qualifications

• Bachelor's degree in Human Services or related field preferred but not required.
• MHRTc certification or eligible.
• Valid state driver's license required.

Responsibilities

• Conduct assessments with participants and guardians for health and social service needs.
• Provide information on primary and emergency care and explore less restrictive options to hospitalization.
• Coordinate and monitor the Individual Treatment Plan (ITP) with the integrated team.
• Refer participants to additional services including housing, food, clothing and other basic needs.
• Be available for in-person coverage at the CCBHC walk-in center.
