Job summary

Hope Network is seeking a Case Manager Clinician to provide therapeutic services, case management, and care coordination for individuals receiving services. You will work with an interdisciplinary team to create person-centered treatment plans and connect clients with resources to foster independence.

You will conduct psychosocial assessments, provide therapy, coordinate IPOS, and support transitions of care while maintaining thorough documentation.

Qualifications

• Master's degree required in a relevant field.
• Current licensure or eligibility for licensure in Michigan (LMSW/LLMSW, LP/LLP/TLLP, LLC/LPC).
• Ability to participate in an on-call rotation as required by program needs.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
• Provide individual and group therapy services.
• Coordinate PCP meetings and IPOS.
• Monitor consumer progress and coordinate services with the interdisciplinary team and community partners.
• Support consumers through admissions, discharges, and transitions of care.
• Collaborate with nursing staff, guardians, CMH, and other providers for coordinated care.
• Respond to clinical emergencies and participate in crisis intervention and related legal proceedings.
• Complete timely clinical documentation and maintain accurate records.
