Qualifications

• Knowledge of social case work principles and methods.
• Knowledge of addiction as a disease and its treatment.
• Knowledge of current social, economic and health problems.
• Ability to establish and maintain an effective working relationship with clients from various cultures and social economic backgrounds.
• Ability to communicate effectively, both orally and in writing.

Responsibilities

• Advocates for patients and assists with all aspects of health by coordinating with providers and team members.
• Learns assessment practices to identify service needs from clients, families, and collaterals.
• Creates partnerships with community resources for patient-centered care.
• Coordinates access to addiction, housing, childcare, medical and financial services as needed.
• Maintains confidentiality and accurate patient records.
• Monitors treatment progress and conducts follow-ups on referrals.
• Documents services and supports quality of care through COE standards.
