Qualifications

• Bachelor’s degree includes or is supplemented by 18 credits in sociology, social welfare, psychology, criminal justice or related social sciences with personal recovery experience.
• Associates degree in a social science field with any equivalent combination of experience and training with lived recovery experience.

Responsibilities

• Advocates for and assists patients with physical and mental health by collaborating with medical providers and internal teams.
• Learns assessment practices to identify service needs and establishes personal rapport with patients and relevant collaterals.
• Maintains relationships with community partners to support patient-centered care and access to services.
• Documents patient care services by charting in patient records and ensures confidentiality and privacy.
