Job summary

Trilogy, Inc. in Rogers Park, Chicago, is seeking a Recovery Counselor (Case Management; Mental Health / Social Work) to assess clients and plan treatment through home, community and office visits.

You will provide client-centered, strengths-based care and 75% community-based services to support recovery from severe mental illness and co-occurring disorders. The role requires licensure/certification as listed or equivalent experience, plus crisis coverage and documentation in EMR.

Qualifications

• Must have one of the listed qualifications for eligibility.
• Valid Illinois driver’s license with daily vehicle access is required.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situations via home, community, and office visits.
• Coordinate establishment of an individual recovery plan with client and support system.
• Complete daily progress notes per policy and standards.
• Document care in the EMR and complete required forms (IMCANS, LOCUS).
• Provide crisis on-call coverage as directed.
• Advocate for clients and link to community services as needed.
• Coordinate with team nurse/pharmacy to ensure medication accuracy.
• Assist with entitlements applications (SSI, SSDI, Medicare, Medicaid, LINK).
• Assist clients in learning independent living skills and housing support.
• Transport clients to appointments and provide housing/substance use support.

Skills

Valid Illinois driver's license

Experience with mental health and/or S

Education

High School Diploma or GED

Associate’s or Bachelor’s degree

Licensed Practical Nurse under the Nurse Practice Act

Certificate of Psychiatric Rehabilitation from a DHS-approved program

CRSS certification

Family Partnership Professional certificate from IAODAPCA

Occupational Therapy Assistant licensed

Five years of supervised clinical experience in a mental health setting
