Responsibilities

• Provide intake, assessment and case management services to clients.
• Assess program eligibility and monitor care and client services.
• Educate clients on rights, responsibilities and appeal rights.
• Utilize case management skills to link clients and families to resources.
• Develop and revise client care plans with client input.
• Schedule and participate in case conferences and discharge planning.
• Maintain client files and document case notes.
