Job summary

Qualifications

• Total work experience can substitute for the combination of education and work experience; five years of total experience if needed.

Responsibilities

• Develop a comprehensive plan of care that meets the needs of the client.
• Provide the client with information on community services and agency referrals addressed to client needs.
• Educate clients on program participation including rights, responsibilities and appeal rights.
