  • Bachelor’s Degree in Criminal Justice or Social Science with at least 1 year of relevant experience.
  • Develop a comprehensive plan of care that meets client needs.
  • Provide referrals to community and agency services for clients.
  • Educate clients on program participation, rights, responsibilities, and appeal rights.
  • Maintain care plans, case notes, and related documentation.

Responsibilities

  • Develop, implement, and manage a program for each resident/caseload as per contract requirements.
  • Coordinate case conferences with clients and caregivers and negotiate care plans.
  • Provide discharge planning to ensure clients’ needs are met upon disenrollment.
  • Maintain client files and ensure services are supported by care plans.
  • Transport residents and participate in mandatory trainings as required.
