Indianhead Community Action Agency, Inc. is seeking a Case Management Specialist to deliver comprehensive case management services and coordinate referrals, intake, eligibility, planning, monitoring, and evaluation for clients.

The role emphasizes data accuracy, file maintenance, and collaboration with partner organizations to meet grant requirements. Excellent communication and computer skills are essential, as is the ability to travel as needed.

Qualifications

• Knowledge of case management and crisis management principles.
• Strong organizational and time-management skills.
• Ability to document and report program activities clearly.

Responsibilities

• Provide Case management services.
• Coordinate aspects of case management including referral, intake, eligibility, planning, monitoring, assessment, and evaluation.
• Monitor and record clients' progress to meet goals and objectives.
• Maintain properly organized client files and documentation.
• Maintain professional relationships with partner organizations and service providers.
• Direct case service allocations and participate in related committees.
• Collect and enter performance measure data as required by grants.
• Conduct follow-up calls/meetings with exited members monthly or quarterly as applicable.
• Travel as required and adhere to program guidelines from funding sources.
