Job summary

Banner Health is seeking a Behavioral Health Case Manager to provide comprehensive care coordination across the patient care continuum. You will assess plans of care, implement and monitor resource utilization, and document progress to meet payer and regulatory requirements.

The role emphasizes collaboration with physicians, payers and an interdisciplinary team, guiding discharge planning and educating staff on managed care concepts.

Qualifications

• Master's degree in social work or related field.
• 5 years of clinical experience as required by state law.
• Licensure in at least one of social work, professional counseling or marriage and family.
• Board licensure in addictions for substance abuse when providing direct services.
• Knowledge of care management, hospital and community resources.

Responsibilities

• Manage individual patients across the health care continuum to optimize clinical, financial, operational and satisfaction outcomes.
• Provide assessment, psychoeducation, and psychotherapy to individuals, families, and groups; coordinate care from admission to discharge with the treatment team.
• Lead process improvement activities for patient populations to improve outcomes across domains.
• Evaluate medical necessity and optimize patient outcomes.
• Establish collaborative relationships with physicians, payers and the health care team; communicate information to fulfill requirements.
• Lead interdisciplinary discharge planning and communicate the plan across care settings.
• Educate health care team on case management and managed care concepts.
