Job summary
DuPage County Department of Community Services is seeking a Case Manager in Adult Protective Services. You will conduct comprehensive assessments, determine eligibility, develop service plans, and provide ongoing follow-up and documentation.
Requirements include a BA/BS in Social Science, Human Services, Social Work or related field (or RN/LPN) plus 1 year of assessment experience and a valid Illinois driver's license. Spanish fluency is a plus; benefits include medical, dental, vision and more.
Qualifications
- BA/BS in Social Science, Human Services, Social Work, or related field, or RN/LPN.
- At least 1 year of assessment or service provision experience.
- Valid Illinois Driver’s License.
Responsibilities
- Conducts a comprehensive client assessment in person to identify needs.
- Determines eligibility for services.
- Promotes available services and assists with decisions.
- Develops service plans.
- Provides follow-up and ongoing case management.
- Completes mandated paperwork accurately and timely.
- Uses computer programs for daily tasks and reports.
- Performs other related duties as required.
