Job summary

DuPage County is seeking a dedicated Case Manager within the Department of Community Services to coordinate services for adults and families. You will perform comprehensive assessments, develop service plans, and provide follow-up to ensure successful outcomes.

The role emphasizes accurate documentation and effective communication with clients and partners. Required qualifications include a BA/BS in a related field or RN/LPN, with at least one year of assessment experience and a valid Illinois

Qualifications

B.A./B.S. in Social Science, Human Services, Social Work, or RN/LPN.
At least 1 year of experience in assessment or service provision.
Valid Illinois driver’s license.
Some programs may allow years of experience to substitute education.

Responsibilities

Conducts a comprehensive assessment by interviewing the client in person to assess needs.
Determines clients’ needs and eligibilities for services.
Develops service plans and provides follow-up and case management.
Completes mandated paperwork accurately and in a timely manner.
Utilizes various computer programs for daily tasks and reports.
Performs other related duties as required.
