Job summary

DuPage County Department of Community Services is seeking a Case Manager for the Family Self-Sufficiency program. The role provides comprehensive case management to assist low-income households, assess needs, enroll in grant-funded services, and develop plans toward economic self-sufficiency.

The position requires a BA/BS in Social Science or related field, or RN/LPN, and at least one year of assessment or service provision experience.

Qualifications

• Completion of a BA/BS in Social Science, Human Services, Social Work, or related field, or RN/LPN.
• One year of experience in assessment or service provision.
• Must possess and maintain a valid Illinois Driver’s License.
• Pre-employment background check is required.

Responsibilities

• Conduct a comprehensive client assessment via in-person interview.
• Determine client needs and eligibility for services.
• Develop service plans and provide follow-up, monitoring, and case management.
• Complete mandated paperwork accurately and timely.
• Utilize computer programs for daily tasks and reports.
• Perform other related duties as required.

Job description

We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. The current Case Manager opening is in the following programs/assignment area:

Family Self- Sufficiency

Provides Comprehensive case management to assist low-income households stabilize their housing situation, meet basic needs, access mainstream services, increase income and develop plans and goals toward economic self-sufficiency. Assesses and enrolls households in available grant funded services. Provides follow up services. Works collaboratively with community partners. Family Self-Sufficiency requires home and community visits and some evening hours.

This position is dependent on available grant funding.

Requirements include the following experience or equivalent combination of training and experience:

• One (1) year of experience in assessment or service provision
• Certain programs may allow years of experience to replace educational requirements
• Must possess and maintain in good standing, a valid Illinois Driver’s License
