Job summary

American Addiction Centers seeks a dedicated Case Manager to coordinate treatment services and advocate for patients across the care continuum. You will manage caseloads, document care plans, and collaborate with counseling and UR teams to support recovery goals.

This role requires CADC II or higher certification, a behavioral science degree, and at least 2 years in behavioral health. Strong communication, EHR proficiency, and adherence to HIPAA are essential for success.

Qualifications

• Bachelor’s Degree in Behavioral Sciences preferred.
• State recognized CADC II or above required.
• Minimum of 2 years in behavioral healthcare or substance abuse recovery.
• Experience in behavior health treatment including group facilitation and case management.
• Knowledge of using electronic health record software and documentation.
• Knowledge of ASAM criteria.
• Ability to read, write and interpret information; capable of presenting to groups.
• Proficient in Word, Excel, and Outlook.

Responsibilities

• Perform all case management requirements in accordance with scope and state regulations.
• Maintain a patient caseload.
• Provide and document weekly coordination of care to improve treatment adherence.
• Complete aftercare assessment, discharge plans, and contacts.
• Maintain communication with the referent, family, and external agencies as needed.
• Facilitate life skills and recovery groups.
• Provide weekly sessions addressing recovery stability and discharge planning.
• Maintain confidentiality and HIPAA compliance.
• Participate in team meetings and ensure quality care.
