Job summary

Nulton Diagnostic and Treatment Center (NDTC) seeks a Blended Case Manager to provide supportive case management for mental health consumers in Pennsylvania. Candidates may hold a Bachelor's degree in related social sciences, or be a Registered Nurse, or have a high school diploma with 12 credits and two years of relevant experience.

Qualifications

• Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
• Registered nurse
• High school diploma holders with 12 credits in social science and two years of direct contact with mental health consumers

Responsibilities

• Provide blended case management to mental health consumers.
• Collaborate with healthcare providers to coordinate services.
• Support clients in accessing community resources and benefits.
