Job summary

Nulton Diagnostic and Treatment Center (NDTC) in Greensburg, PA is seeking a Blended Case Manager to join our mental health team. You will coordinate care, support recovery goals, and help clients access community resources.

• Bachelor's degree with major coursework in sociology, social welfare, psychology, gerontology, or other related social sciences.
• Registered nurse.
• High school diploma with 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years experience in direct contact with mental health consumers

Education

Bachelor's degree with major coursework in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education

Registered nurse

High school diploma with 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years experience in direct contact with mental health consumers

Starting Pay and Compensation:

• Base weekly pay plus bonus
• For hours over 27 billable hours weekly: $30 bonus per hour
• For billable hours beyond 32 weekly: $40.00 bonus per hour
• Annual starting earnings potential, including base pay and additional incentives, starting pay ranges from $40,000 to $50,000. This total encompasses all aspects of compensation, reflecting our commitment to rewarding our team's hard work and dedication.

Requirements:
