Department of Health and Social Services (DHSS) in Delaware is seeking a Behavioral Health Case Manager II to assist with intake, assessment, and case management for Mental Health court clients in the community and DOC. The role involves urine monitoring, front desk coverage, and referrals to appropriate care levels to support TASC and related stakeholders.

Applicants must have a Bachelor's degree (or higher) in Behavioral or Social Science or a related field and relevant experience in case

Formación

• Bachelor's degree or higher in Behavioral or Social Science or related field.
• Six months' experience in case management.
• Six months' experience in providing clinical treatment, counseling or determining eligibility for services or benefits.
• Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
• Six months' experience in narrative report writing.

Responsabilidades

• Assists with intake/referral, assessment, and case management of Mental Health court clients referred by Delaware Courts.
• Urine monitoring and front desk coverage.
• Refer clients to appropriate care levels and follow client progress.
• Prepare court reports and narratives; coordinate with multi-disciplinary teams.

Bachelor's degree in Behavioral or Social Science or related field
