Job summary

Vista Village in Columbus, OH seeks a compassionate Case Manager to support participants in a Christian faith-based transitional housing program. You will assess needs, develop service plans, coordinate community resources, and document progress with a person-centered, trauma-informed approach.

The role emphasizes advocacy, collaboration, and meeting benchmarks, with a schedule Monday–Thursday and Friday half-day; compensation ranges $50,000–$60,000, plus benefits and ongoing training.

Qualifications

• Bachelors or equivalent experience in social services or housing
• Experience in case management or behavioral health settings preferred
• Experience with homelessness, mental health, or substance use disorders preferred
• Knowledge of local community resources and behavioral health systems preferred
• Strong written and verbal communication with accurate documentation
• Valid driver's license and reliable transportation

Responsibilities

• Conduct intake and bio-psychosocial assessments and orient participants to program requirements
• Develop, implement, and update individualized service plans
• Help participants set realistic goals and identify steps to achieve them
• Conduct routine counseling sessions with assigned caseload
• Document progress toward program benchmarks and prepare notes and plans
• Connect participants with community resources, benefits, housing, employment, and education
• Maintain confidential case files and required documentation
• Facilitate life-skills sessions and psychoeducational activities as needed
• Advocate for participants and promote self-sufficiency and accountability
• Participate in team meetings, case consultations, and program reporting
• Perform other duties related to participant support and operations
