Job summary

Vesta, Inc is a not-for-profit mental health organization in Maryland seeking a Case Manager for the Forestville office to support the Southern Regions. You will coordinate services, conduct assessments, and help clients access education, job training, and community resources to achieve their goals.

The role involves maintaining records, coordinating somatic health referrals, and liaising with care teams, with opportunities for some evenings and weekends as needed.

Qualifications

• 3 years of experience in Case Management.
• Experience working with clients with mental health disorders.
• Ability to pass background check and drug screening.

Responsibilities

• Provide supportive counseling and advocacy for clients.
• Assist with client assessments and provide information on life aspects.
• Facilitate access to educational and job training opportunities.
• Maintain accurate case records in compliance with policy and regulatory requirements.
• Refer clients to appropriate services and support groups.
• Conduct on- and off-site meetings with clients as required.
• Act as a liaison for hospitalized clients and follow up on somatic care referrals.
