This full-time, exempt role supports the PRP program in the Southern Region and involves outreach, assessments, and advocacy. The ideal candidate has a graduate degree in health and human services or a BA with relevant experience, at least 3 years in case management, and a valid driver’s license.

Qualifications

• Graduate degree in health and human services preferred; bachelor's degree with relevant experience considered.
• 3 years of experience in case management.
• Experience working with clients with mental health disorders.
• Valid driver's license and reliable transportation; willingness to transport clients.
• Ability to pass background check and drug screening.

Responsibilities

• Provide supportive counseling and advocacy for clients.
• Assist with client assessments and connect clients to life resources.
• Facilitate access to educational and job training opportunities.
• Maintain accurate case records in compliance with policies and regulations.
• Refer clients to appropriate services and support groups.
• Conduct on-site and off-site meetings and groups as needed.
• Act as liaison for hospitalized clients and follow up on somatic care referrals.
