The role includes travel, documentation, and collaboration with guardians and community partners.

Qualifications

• Experience with case investigation and social work practices.
• Ability to travel extensively to provide services.
• Ability to write comprehensive progress notes and plans.
• Knowledge of state statutes related to elder maltreatment.

Responsibilities

• Investigate and provide assessment and services for complex cases involving vulnerable and incapacitated adults.
• Perform in-home assessments and arrange appropriate services.
• Prepare case plans, risk assessments, and closure summaries.
• Coordinate with guardians, conservators, and community resources.
